Please complete the Meeting Registration Form so that we may plan your attendance and bill the USM institution for the meeting fee.

i) Conference Fee of $65pp includes AM break, lunch, PM break, breakfast on Tuesday and materials.
ii) Assuming that you have accommodations somewhere, we can accept Meeting Registrations for the ICC Fall 2016 Retreat until Fri. Oct. 14th at 12noon.
iii) The deadline date for cancelling your Conference Fee, in the event that you are unable to attend the retreat, is Oct. 10, 2016.
